Like all tanning facilities in Ohio, there are a few forms that you will need to complete before UV or Sunless tanning with us. Providing some of the forms here are meant to cut down time for your first visit. Plus, eliminating some of the initial paperwork give us more time to show you around, talk about your tanning goals, and make you feel part of the family!
